What is an electronic signature?
An electronic signature is an electronic sound, symbol, or process, attached to or logically associated with a contract or other record and executed or adopted by a person with the intent to sign the record. You can use electronic signatures on documents requiring your signature. By using electronic signatures you eliminate the need to print, sign and fax your documents to us.
Do I have to use electronic signatures?
No, you do not have to use electronic signatures. If you would prefer, you can simply select the link “Print, sign and fax” at the bottom of the screen to sign and submit your documents.
Is it a legal signature?
Yes, since 2000, federal legislation ensures that electronic signatures are the equivalent of a pen-and-ink signature. Using our electronic signatures is the same as signing the document by hand.
Is it secure?
Yes, it is secure. To ensure the integrity of your electronic signature, the Student Password and identifying information that you are asked to provide will be validated. All sensitive information is encrypted and sent over 256 bit SSL connections to ensure extra protection for your private information.
What is the Password that I am being asked to enter?
It is the same password that you use to log in to your student account on our school's website.
Troubleshooting Tips:
If you try to submit the signature and receive an error message, or you get no notification of the submission, this could be a sign that the electronic signature is not working properly. If this occurs, please contact Tech Support at (847) 585-2268 so we can troubleshoot the issue.
This Electronic Signature Notice asks for your consent in order for our school to provide you with information in electronic form and to receive your signature in electronic form on any document requiring your signature. If you determine that you no longer wish to receive communications electronically, you may withdraw your consent at any time by calling, e-mailing or writing. If you withdraw your consent, the terms of any previously delivered and executed documents in electronic form will continue to apply to you and your related transactions with us. Future communications and transactions will be conducted in paper form. Please note that if you elect to receive communications and transactions in paper form, delays may be experienced as a result of U.S. mail delivery and manual processing. You have the ability to receive a paper copy of this disclosure statement and any other document. The option will always remain to print a paper-based version of your forms or communications on demand in its identical format. Medical Documentation: Medical documents are required on office letterhead or prescription pad and should meet all of the following criteria:
Example: Letter should state something similar to "John Doe needs a leave of absence from SCHOOL from (Input Start and Return Date) due to medical reasons" (Cannot accept if it says for work or anything else);
Family Care - Medical Documents: Documents can be in the form of a doctor's note or form (as long as it has all the below information).
Example: "John Doe is the primary caretaker for Jane Doe. John Doe will need to take a leave of absence from school starting (Input Start and Return Date)."
Family Care - Death of a Family Member: This is covered under the Family Care portion of the LOA policy. Documents are required to be in the form of a death certificate or an official letter from a doctor;
If in the form of a letter from a doctor, the letter should state the following:
Jury Duty: Documentation is required in the form of the official summons for jury duty and needs to include the following information:
Military: If a member of the military or their spouse is being deployed, relocated, or sent to training they will have paperwork that will be sent to them.
